Clinical Pharmacist Services

Porirua Union & Community Health Service (PUCHS) offers comprehensive clinical pharmacist services to residents across the wider Porirua area, including Johnsonville and Paremata. These services are designed to enhance medication management, promote safe and effective use of medicines, and support overall health and well-being.

CLINICAL PHARMACIST SERVICES:
Medication Reviews: Clinical pharmacists conduct thorough reviews of patients' medications to ensure they are appropriate, effective, and safe. This process helps identify potential drug interactions, side effects, and opportunities for therapy optimization.

Patient Education: Pharmacists provide education to patients about their medications, including proper usage, potential side effects, and the importance of adherence to prescribed regimens. This empowers patients to take an active role in managing their health.

Chronic Disease Management: For individuals with chronic conditions such as diabetes, hypertension, or asthma, clinical pharmacists work with the healthcare team to develop and implement personalised medication plans that help achieve the best health results.

Support for Complex Cases: Clinical pharmacists assist in managing complex cases involving use of multiple medications, ensuring that treatment plans are both effective and safe for patients.
