Community Support Team

Our Community Health and Social Services team is here to support individuals and whānau with health, wellbeing, and social needs – and the best part? It's all free. The Health Coach (HC) can guide you through lifestyle changes and health goals, while the Health Improvement Practitioner (HIP) helps create a personalised care plan to improve your overall health. The Community Health Worker (CHW) is available to help you navigate the healthcare system and provide educational support. Our Cross-Cultural Worker (CCW) works closely with our refugee community, offering culturally appropriate assistance and helping bridge any communication gaps. No matter what your needs are, our caring team is here to provide coordinated, holistic care, both in the clinic and at home, at no cost to you.

Providing support services for individuals and whānau facing mental health challenges. Our team offers counselling, assessments, and tailored care plans to promote well-being and resilience.

Clinical Pharmacist

Our pharmacist works closely with patients and healthcare teams to review medications, optimise therapy, and provide education and advice on proper use.
