At Porirua Union & Community Health Service (PUCHS), our nurse-led diabetes clinic provides dedicated support to help you take control of your health. We offer extended appointments with our experienced diabetes nurse, ensuring you receive the time and attention needed to manage your condition effectively.
As part of the comprehensive care we provide, people may be eligible to see a podiatrist, dietitian, clinical pharmacist, or general practitioner (GP), depending on their needs. The clinic also focuses on self-management education, empowering patients with the knowledge and tools to effectively manage their condition.
Additionally, we hold specialist case conferencing with an endocrinologist to gain expert advice and remove barriers to care for our patients, ensuring a holistic and accessible approach to diabetes management.
We’re committed to community-focused, culturally responsive care. Recently PUCHS has partnered with Otago University researchers to better understand Pacific families’ experiences with diabetes, helping us refine our approach to meet the unique needs of our community.
