Outreach Service

At Porirua Union & Community Health Service (PUCHS), we know that not everyone can easily access a medical clinic. That’s why our community outreach services bring healthcare and support
directly to those who need it most—elderly individuals, people with mobility challenges, and those requiring in-home assessments.

Our nurse-led outreach service provides in-home health assessments, chronic condition management, medication support, and general health checks, ensuring quality care for those with limited mobility or other challenges.

Beyond nursing care, our wider outreach team—including a health coach, community worker, cross-cultural worker, and Health Improvement Practitioner (HIP)—offers additional support in the community. Whether it’s mental health guidance, lifestyle coaching, social support, or cultural advocacy, our team is here to help people access the care they need in a way that works for them.

By delivering care beyond the traditional clinic setting, PUCHS ensures that healthcare in Porirua is accessible, equitable, and tailored to the needs of our diverse community.

Offering practical, holistic support to help individuals and whānau navigate life’s challenges and improve wellbeing. Our team includes a Health Coach, Community Health Worker, Health Improvement Practitioner, and Cross-Cultural Worker, working alongside you to achieve your goals.

Mental Health

Providing support services for individuals and whānau facing mental health challenges. Our team offers counselling, assessments, and tailored care plans to promote well-being and resilience.

Clinical Pharmacist

Our pharmacist works closely with patients and healthcare teams to review medications, optimise therapy, and provide education and advice on proper use.
